ENHANCED CARE MANAGEMENT (ECM)

High Touch, whole-person care management
Free for eligible MediCal Members

Health Projects Center’s Enhanced Care Management (ECM) helps people receiving MediCal to effectively manage their care.

We can help you get healthy and stay as healthy as possible.

What are ECM services?

 

If you qualify for ECM, you will be assigned a care manager who will work to help you access and coordinate services from doctors, counselors, specialists, pharmacists, social service providers and others. 

The care manager will work to make sure everyone works together to get the care you need.  They will help you to assess your needs and to find and apply for available community services.  They will be your advocate and coach within the health and social services system.

An ECM lead care manager can help you:

  • Find doctors

  • Schedule appointments for services

  • Find transportation to access health care

  • Find and apply for community and government services

  • Access nutritious food and appropriate housing

  • Set up in home follow up care after a hospital stay

  • Access health education

How can I get ECM services?

​Health Projects Center works in partnership with the Central California Alliance for Health to help you access ECM services.  Health Projects Center focuses on supporting MediCal members who are at risk for placement for long-term care services without support.
