Health Projects Center’s Enhanced Care Management (ECM) helps people receiving MediCal to effectively manage their care.
What is Enhanced Care Management (ECM)?
Enhanced Care Management (ECM) is a no-cost program designed to assist individuals experiencing complex health issues or social needs. If you qualify for the program, a dedicated care team will collaborate to coordinate services and resources to help alleviate barriers impacting your health and well-being.
These services may include medical support, social services, and access to community resources. ECM is offered as part of your current Medi-Cal plan.
Who may be eligible for ECM services?
You may qualify for ECM services if you:

Have had three or more unplanned hospital stays or five or more emergency room visits within a six month period

Are actively experiencing at least one complex social or environmental factor that is affecting your health. Examples include, but are not limited to:

Needing assistance with activities of daily living (ADLs)

Experiencing communication challenges

Limited access to food or stable housing

Living alone without adequate support or experiencing caregiver challenges

Needing a conservator or guided decision-making support

The Enhanced Care Management (ECM) program is designed to create a dedicated care team that will work with you to:
