What is Care Management?

 

Care management enables adults to maintain their independence at home through personalized care coordination and support. This includes our Multipurpose Senior Services Program (MSSP), Enhanced Care Management (ECM), and the Medi-Cal Waiver Program (MCWP), which connect individuals to essential services that promote safety and well-being.

 

Care management is a team effort, often facilitated through care managers. Care managers can help recipients access and coordinate services from care providers, community organizations, and social services.

 

What are the goals of care management?

• Focus on individualized, whole-person care 

• Maintain independence and quality of life at home

• Coordinate care across health and social services

• Reduce barriers to accessing care
