What is a Community Health Worker?
A Community Health Worker is a trusted community advocate who connects members with healthcare and social services. With strong knowledge of the community, they help you access services that align with your language and cultural needs.
CHWs provide education, personalized guidance, and support in linking members to appropriate resources. While they do not provide licensed medical care or services such as transportation, personal care, homemaking, or childcare, they can help identify and connect you with programs that offer these types of assistance.
Who may be eligible for CHW services?
You may qualify for ECM services if you:

Have appropriate Medi-Cal
A CHW may be able to help you with the following services:

Health Education

Health Navigation

Transportation

Individual support or advocacy.
