Message from the Director
For more than 46 years, Health Projects Center has worked to improve the health and well-being of people across California's Central Coast. I am proud to share that this past year, we served more individuals through more programs than at any other time in our organization's history.
Every day, our team helps people with complex health and social needs remain safe in their homes with dignity, independence, and access to care. Through care management, caregiver support, and healthcare workforce development programs, we provide essential services made possible by partnerships with public agencies, private funders, donors, and community supporters.
Looking ahead, we face significant challenges. Coming reductions in federal government healthcare funding, particularly through Medi-Cal, threaten access to critical services for many of our community's most vulnerable residents. Individuals and families who rely on these programs to maintain their health, stability, and quality of life will feel the impact.
HPC is prepared to meet this challenge. Guided by our mission and strengthened by the dedication of our staff, volunteers, partners, and supporters, we will continue working to fill gaps in care and connect people with the resources they need. Nonprofit organizations play a vital role when community needs exceed available public resources, and we are committed to being part of the solution.
Please join us in this effort. Whether you volunteer your time, advocate for policies that strengthen our communities, or make a financial contribution, your support helps ensure our neighbors receive the care and compassion they deserve.

HPC's mission is crucial in providing exceptional care to our aging population, helping them remain safe and comfortable at home, and easing financial demands on the state.
We are committed to thoughtful growth and adapting as needed while staying dedicated to those we serve. With ongoing support from our partners, we are well-positioned to continue making a meaningful impact.

Who is Health Projects Center?
Health Projects Center is a 501(c) (3) nonprofit corporation that has supported Californians living and aging safely at home since 1980. We deliver high-quality services and programs focused on care management, caregiver support, and workforce development.
Serving Monterey, San Benito, Santa Cruz, and San Luis Obispo Counties, Health Projects Center empowers individuals and families to navigate the challenges of aging with dignity and confidence. We deliver culturally appropriate programs in both English and Spanish for older adults, family caregivers and health workers.
Each year, our programs help over 2,200 families live and age at home at a fraction of the cost of institutional care.

Established Del Mar Caregiver Resource Center to serve families caring for brain-impaired loved ones.
2012
Expanded MSSP care management services into Monterey County.
2025
Expanded MSSP and ECM operations into San Luis Obispo County.

Founded as the Central Coast Area Health Education Center (AHEC) to train healthcare professionals in geriatrics.
Awarded contract to operate Santa Cruz County's Multipurpose Senior Services Program (MSSP).
Launched Enhanced Care Management (ECM) to provide high-touch care for complex Medi-Cal members.
Launched Community Health Worker (CHW) program to directly address social
determinants of health.

During fiscal year 2025 - 2026, our services helped 2,294 individuals remain in the community and avoid long-term care.

If all of our efforts delayed placing 58 people into skilled nursing by one year, it would save the government and families over $8 million—the full annual HPC budget.
