Helping people living with HIV/AIDS to maintain their independence at home.
The Medi-Cal Waiver Program (MCWP) supports people living with HIV Disease and AIDS to live safely at home, despite health conditions that put them at risk of institutionalization.
MCWP provides comprehensive care management and direct care services. All services are provided at no cost to the client. Case management incorporates a collaborative interdisciplinary team approach consisting of a nurse case manager and other service.
To be eligible for MCWP you must:

Have a written diagnosis of HIV/AIDS with current symptoms related to HIV disease, AIDS, or HIV disease treatment;

Have health status that is stable enough to make home care appropriate, and a home setting that is safe for the client and service providers;

Have appropriate Medi-Cal eligibility and be certified to meet the Nursing Facility Level of Care or higher as defined in Title 22, California Code of Regulations, Sections 51334 and 51335.

Must be 18 years of age or older.
