Health Projects Center Awarded Multi-Year Grant from Monterey Peninsula Foundation


Santa Cruz, CA - Health Projects Center (HPC) has received a $130,000 multi-year operating grant from Monterey Peninsula Foundation to enhance programs for older adults and individuals with complex health needs. This funding will expand access to essential services and resources for communities in Santa Cruz, Monterey, and San Benito counties.


The grant supports HPC’s three-tiered approach, which integrates direct care management, family caregiver support, and workforce development. Funding will cover essential operational costs for program expansion not fully supported by other sources.


With this funding, HPC will launch mobile outreach in underserved rural areas of Monterey and San Benito, expand support groups for family caregivers, and introduce technology-assisted care options. These initiatives will help HPC reach remote residents and offer programs such as virtual caregiver consultations and personalized care plans.


“This multi-year investment from Monterey Peninsula Foundation is a powerful affirmation of our mission and impact,” said John Beleutz, MPH, Executive Director of HPC. “With this support, we can continue to improve the quality of life for caregivers and their families, prevent unnecessary institutionalization, and expand access to high-quality health services for medically underserved communities.”


Over the grant period, Health Projects Center will:


  • Provide intensive support to over 1,800 family caregivers, improving their well-being through assessments, consultations, workshops, support groups, and paid respite care, which leads to reduced caregiver stress and increased ability to care for loved ones at home.

  • Deliver complex care management to nearly 3,800 medically frail individuals each year, enabling them to remain safely at home and avoid unnecessary hospitalization or institutionalization, resulting in improved health stability and independence.

  • Support nursing students, family practice residents, and in-home supportive services workers with gerontology-focused training and hands-on experience, increasing the local workforce’s capacity to provide specialized care for older adults.
