Health Projects Center Launches Innovative Fall Prevention Program in Monterey County

Monterey, CA — Health Projects Center (HPC) is proud to announce the launch of a groundbreaking fall prevention initiative, aimed at bolstering the safety and independence of older adults in Monterey County. With generous grant funding, HPC is set to provide complimentary fall prevention home assessments for low-income older adults, helping them to live securely within their own homes.


HPC's dedicated case managers will evaluate participants' homes to identify potential fall hazards and coordinate with local resources to address them. This program is designed to facilitate essential home modifications at no expense to the participants, thanks to the support provided by the grant. These modifications include the installation of grab bars, repairs to stairs and banisters, and lighting fixture updates, all conducted by licensed​ contractors.


"Our fall prevention program is a testament to our commitment to the well-being of older adults in our community," stated Maria Ramirez, ASW, Supervising Social Work Care Manager at HPC. “We’re excited to launch this initiative, which provides education and possible vital safety modifications that empower older adults to remain independent in their homes. With this funding, we can take meaningful steps to reduce falls and help seniors live comfortably and safely in familiar surroundings.”


This life-enhancing program is available on a first-come, first-served basis and specifically targets older adults committed to aging in place but require additional support to ensure their safety.


Eligible individuals must be:

  • 60 or older

  • Low-income

  • Monterey County Resident

  • Not receiving institutionalized care, but aging in place with support and supplemental services

  • Requires modifications to homes to continue aging in place


With 15.5% of Monterey County's population being 65 years or older, the need for such a program is clear. HPC’s Fall Prevention program addresses the immediate safety concerns associated with falls and supports the goal of enabling old adults to age with dignity in their homes.


Referrals for the program are currently being accepted, and qualifying residents of Monterey County are urged to apply as soon as possible to benefit from this unique opportunity.
