As we enter 2025, it is an opportune moment to reflect on our achievements, challenges, and the path ahead. This year brings new opportunities for growth and innovation, and I am excited to share our vision for the future. Together, we have the potential to make a significant impact, and I am grateful for your continued commitment and collaboration. Let us embark on this journey with renewed energy and purpose.


California’s government-funded health insurance for low-income people, Medi-Cal, is in the midst of a historic transformation aimed at providing comprehensive preventive and personalized care to more effectively address Californian's physical and mental health needs.  


California Advancing and Innovating Medi-Cal (CalAIM) is an initiative central to this healthcare transformation. CalAIM, a multi-year plan for improving Medi-Cal, will standardize, simplify, and streamline how recipients access health care. 


CalAIM delivers new services that help people address social and economic needs that can impact their health, like help getting and keeping housing and accessing medically tailored meals to support a member’s short-term recovery. These services are a game changer—helping address key social and economic issues that contribute to better health outcomes.   


Health Projects Center is embracing CalAIM and is collaborating with health plans, care providers, and other community-based organizations to make this vision a reality.   


Health Projects Center has implemented Enhance Care Management (ECM), a key component central to CalAIM, in Monterey, Santa Cruz, and San Benito Counties for eligible members with complex needs. 


ECM provides access to a dedicated care manager who coordinates care and services for the member and breaks down the traditional walls of health care, extending beyond hospitals and healthcare settings into communities by providing in-person care management to people with high needs where they live.   


ECM care managers listen to the input from the participant and their clinical team to develop a person-centered care management plan. Care managers organize activities per a care management plan by partnering with a multi-disciplinary team of care providers.   


ECM care managers engage in a variety of activities to promote participant health including: 


  • Encouraging and supporting ECM participants to make lifestyle choices that support their health and wellness.   

  • Coordinating and accessing mental and physical health care.   

  • Partnering with care facilities to support participants in transferring levels of care.  

  • Supporting families and care providers to address participant’s conditions and overall goals. 

  • Accessing community and social support including CalAIM Community Supports that address members’ health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care.  


In 2024, Health Projects Center delivered ECM to 484 individuals in Monterey, Santa Cruz and San Benito Counties. We anticipate doubling the monthly number of people we serve with ECM over the next two years. To make this happen we have hired a growing team of care managers and invested significant resources in expanding our systems. 


We are proud to be a partner in this CalAIM transformation.
